Death benefit fund enrollment form.

Up until 1955, the Foreign Missions Department offered to AG missionaries opportunity to enrol in a death benefit fund, which upon the death of the missionary would pay to the named beneficiary a sum of $1,000. The enrolled missionary was to pay $1 per month for this death insurance. In 1955, the Foreign Missions Department suspended this coverage and offered the missionaries the opportunity to enrol in the Group Life Insurance Plan offered to ministers.
